Australian model Isabella Carlstrom has confirmed she and her wife Moana Hope have broken up, saying the pair have decided to “consciously uncouple”.

Moana is a former AFLW star and also appeared on Australian Survivor. She and Isabella married in 2019.

The two mums have shared a lot of their relationship on social media, including their journey to parenthood.

But earlier this month Moana changed her Instagram bio from an emoji of two parents and two children to one parent and two children. As a result, many of their followers began speculating the couple were no longer together.

On Monday, Isabella Carlstrom confirmed the sad split on Instagram, devastating their followers.

“It’s with a heavy heart that Mo and I have decided to separate, she wrote.

“We will continue to raise our beautiful kids together as we consciously uncouple.”

“We appreciate all the love and support over the years, thank you.”

Moana Hope hasn’t spoken about the breakup herself. But she said in a recent Instagram video, “My mum would always say just follow your heart. It’s simple but something that has stuck with me forever.”
